What we ask for
The form is short. Most of what a panel needs comes from the evidence you send in the ten working days before the assessment, not from the booking itself.
- The service name, and one sentence on what it lets someone do
- The stage you are booking — alpha, beta, or live
- The service owner's name and role
- Roughly how many transactions a year the service expects
- Your preferred month, and any date you cannot make
