Public records request form (ODS-14)
The paper form for requesting your public records, for anyone who cannot or does not want to use the online service.
When to use this form
Use this form if you would rather not make your request online, or if you do not hold a Commonwealth ID account. The online service is usually faster, but this route carries exactly the same statutory reply time of 20 working days.
You can also make a request by telephone on 0800 118 4401, and someone will complete the form with you.
What you will need
You will need to prove who you are. Send a copy of one photographic identity document with the form — we return it within five working days of receiving it.
- Your full name, date of birth, and current address
- Any previous names or addresses relevant to the records
- Which departments you believe hold the records, if you know
- A copy of one photographic identity document
